RESUMO

OBJECTIVE: To define the relationship between chronic chikungunya post-viral arthritis disease severity, cytokine response and T cell subsets in order to identify potential targets for therapy. METHODS: Participants with chikungunya arthritis were recruited from Colombia from 2019-2021. Arthritis disease severity was quantified using the Disease Activity Score-28 and an Arthritis-Flare Questionnaire adapted for chikungunya arthritis. Plasma cytokine concentrations (interleukin (IL)-1ß, IL-2, IL-4, IL-6, IL-8, IL-10, IL-12p70, IL-13, interferon-γ and tumor necrosis factor (TNF)) were measured using a Meso Scale Diagnostics assay. Peripheral blood T cell subsets were measured using flow cytometry. RESULTS: Among participants with chikungunya arthritis (N = 158), IL-2 levels and frequency of regulatory T cells (Tregs) were low. Increased arthritis disease activity was associated with higher levels of inflammatory cytokines (IL-6, TNF and CRP) and immunoregulatory cytokine IL-10 (p<0.05). Increased arthritis flare activity was associated with higher Treg frequencies (p<0.05) without affecting T effector (Teff) frequencies, Treg/Teff ratios and Treg subsets. Finally, elevated levels of IL-2 were correlated with increased Treg frequency, percent Tregs out of CD4+ T cells, and Treg subsets expressing immunosuppressive markers, while also correlating with an increased percent Teff out of live lymphocytes (p<0.05). CONCLUSION: Chikungunya arthritis is characterized by increased inflammatory cytokines and deficient IL-2 and Treg responses. Greater levels of IL-2 were associated with improved Treg numbers and immunosuppressive markers. Future research may consider targeting these pathways for therapy.

BACKGROUND: While edentulism remains a serious public health problem for older adults in Colombia, few analyses have been conducted from the framework of active aging as a part of the positive discourse of aging. This study analyzed complete edentulism and its relationship with determinants including personal, behavioral, and health systems and social services. METHODS: This study included a total of 19,004 older adults. We used univariate, bivariate, and multivariate logistic regression type scores to investigate the relationships between the variables. The personal determinants included basic (Barthel scale) and instrumental activities of daily living (Lawton scale), public transportation, functional limitations, self-perceived health, and health problems. The behavioral factors included alcohol and tobacco use, mini nutritional tests, and physical activity. The last determinant was the healthcare system, while social services access included dental services. The analysis also included sociodemographic variables. RESULTS: The results revealed significant associations for the variables of the three determinants, including the risk of malnutrition (odds ratio [OR]=1.15), functional limitation (OR=1.15), moderate physical activity (OR=1.08), and access to dental services (OR=2.31). Sex, years of education, and race were also risk factors, among other variables. Personal determinants, behavior, and use and access to health services were related to edentulism in older adults. CONCLUSION: These findings support the need to include different analyses of edentulism from multicausality and to understand the oral cavity and the living conditions of aging adults.

Invasive Candidiasis (IC) and candidemia (as its most frequent manifestation) have become the main cause of opportunistic mycosis at hospital settings. This study, made by members of the Colombian Association of Infectious Diseases (ACIN), was aimed at providing a set of recommendations for the management, follow-up and prevention of IC / candidemia and mucous membrane candida infection in adult, pediatric and neonatal patients in a hospital setting, including the hemato-oncological and critical care units. All the data obtained through an exhaustive search were reviewed and analyzed in a comprehensive manner by all the members of the group, and the recommendations issued are being made after a careful review of the scientific literature available and the consensus of all specialists involved; the emergence of Candida Spp. problem is highlighted and a correct orientation to health professionals regarding the management of patients with candidiasis is provided in a rational and practical way, emphasizing patient evaluation, diagnostic strategies, prophylaxis, empirical treatment, directed treatment and preventative therapy.


La Candidiasis Invasora (CI) y la candidemia, como su manifestación más frecuente, se ha convertido en la principal causa de micosis oportunista a nivel hospitalario. Este manuscrito realizado por miembros de la Asociación Colombiana de Infectología (ACIN), tuvo como objetivo proporcionar un conjunto de recomendaciones para manejo, seguimiento y prevención de la CI/candidemia y de la infección candidiásica de mucosas, en población adulta, pediátrica y neonatal, en un entorno hospitalario, incluyendo las unidades hemato-oncológicas y unidades de cuidado crítico. Todos los datos obtenidos mediante una búsqueda exhaustiva, fueron revisados y analizados de manera amplia por todos los miembros del grupo, y las recomendaciones emitidas se elaboraron luego de la evaluación de la literatura científica disponible, y el consenso de todos los especialistas involucrados, reconociendo el problema de la emergencia de las infecciones por Candida Spp. y brindando una correcta orientación a los profesionales de la salud sobre el manejo de pacientes con enfermedad candidiásica, de una forma racional y práctica, enfatizando en la evaluación del paciente, estrategias de diagnóstico, profilaxis, tratamiento empírico, tratamiento dirigido y terapia preventiva.

RESUMO

We report the emergence of a novel VIM variant (VIM-24) in a Klebsiella pneumoniae isolate in Colombia. The isolate displays MICs for carbapenems below the resistance breakpoints, posing a real challenge for its detection. The blaVIM-24 gene was located within a class 1 integron carried on a large plasmid. Further studies are needed to clarify its epidemiological and clinical impact.

El artículo está dividido en dos partes. En la primera, llamada “las Empresas Sociales del Estado (ESE)”, se trata el marco en que está inscrito este tipo de hospital público descentralizado en Colombia, hasta llegar a la figura del gerente hospitalario, que es el tema del artículo. Se explica como, de acuerdo con lo sugerido por la jurisprudencia de la Corte Constitucional, el mecanismo para elegir gerentes en las ESE a futuro sería exclusivamente el concurso de méritos, lo cual hace que este proceso de selección se haga especialmente relevante y de allí la necesidad de evaluar las experiencias institucionales que se tienen al respecto para refinar los procedimientos a futuro. Partiendo de ésto, en la segunda parte del artículo sobre “Selección de ternas para gerente de ESE en la universidad CES” se hace una reseña histórica del proceso, se explica la metodología que ha venido siendo empleada y se hace un sucinto análisis de lo hecho hasta ahora por la universidad CES en este campo.


The article is divided into two parts. The first one is named “the State Social Enterprises (ESE by the acronym in Spanish)”, and refers on the framework in which is inscribed this kind of public decentralized hospitals in Colombia to finally arrive to the figure of managers of the ESE [hospital chief executive officers], which is the central issue of the article. This part is divided in: decentralization of public hospitals, public funding of ESE, structure and organization of ESE, Directive Boards of ESE, managers of the ESE, evaluation of managers of the ESE, and selection of managers of the ESE. It is explained that, in according what is suggested by the jurisprudente of the Constitutional Court, the mechanism for choosing would be based in future on merit, exclusively, and it could convert the selection process in a particularly relevant step and it justifies to evaluate the institutional experiences to refine the procedures in the future. Since this, the second part of the article, about “selection of triples for manager of ESE in the CES University”, makes a historical review of the processes and explains the methodology that has been used, to finally make a brief analysis about the know-how of the CES University in that field.

El tromboembolismo pulmonar, es una complicacion frecuente en la practica medica, con una letalidad que oscila entre 13 y 58 %, de acuerdo con su gravedad. Es por esta razon, que es necesario conocer las caracteristicas epidemiologicas y clinicas, de los pacientes con esta patologia en nuestro medio. Metodos:se reslizó un estudio descriptivo retrospectivo,en pacientes atendidos en la Clínica Cardiovascular Sabta María de la ciudad de Medellín, durante los años 1996 a 2004. Resultados: en total, se encontraron 94 registros con diagnóstico de tromboembolismo pulmonar confirmado. el 51% de los pacientes, se encontraban hospitalizados. los principales factores de riesgo encontrados, fueron: trombosis venosa profunda, reposo en cama, síndrome de falla cardíaca y antecedente de enfermedad tromboembolitica...


Background: Pulmonary embolism (PE), is a common complication in clinical practice. Mortality is between 13% and 58%, according to severity. Because of that, it is very important to know the epidemiologic and clinic characteristics of patients in our city. Methods: This was a retrospective descriptive study on patients admitted at the Santa Maria Cardiovascular Clinic ofMedellín, between 1994 and 2004. Results: We found 94 charts with confirmed diagnosis of PE, 51% belonged to inpatients.
